- APPLICATION FEES (for membership period: October 1st, 2004 through September 30th, 2005):
- Regular Membership RENEWAL: $75.00 (THROUGH 10/31/04)
- Delinquent Membership RENEWAL: $90.00 (AFTER
10/31/04)
- Junior Membership RENEWAL (17 years-old max.): $15.00
NOTE: Junior membership is for handicap maintenance
purposes ONLY.
Junior members do NOT get to enter club tournaments.
NOTE: Once the application is filled in, you should SUBMIT it,
then PRINT the returned form and mail it, via US Mail, to
the address shown in the header of this page and in the form. Make sure to
include your check for the appropriate amount. DO NOT
e-mail the form back to me. I WON'T save forms e-mailed to me.
